Heparin-Induced Thrombocytopenia (HIT) remains one of the most challenging and high-stakes complications of anticoagulant therapy. Despite its relatively low incidence, HIT is associated with significant morbidity and mortality due to its paradoxical combination of thrombocytopenia and heightened thrombotic risk. This session will explore the evolution of HIT diagnostics from early recognition to today’s rapidly advancing technologies. Participants will gain insight into the clinical presentation and scoring systems that guide pre-test probability, the strengths and limitations of serological and functional assays, and the critical role of laboratory–clinician collaboration in ensuring accurate diagnosis. Finally, the session will briefly highlight automation and emerging innovations—such as chemiluminescent assays and artificial intelligence—that have begun to reshape the diagnostic landscape.

Objectives:
Participants will be able to describe the clinical presentation and pathophysiology of HIT. They will be able to explain the principles, strengths, and limitations of current assays and evaluate the role of automation and emerging technologies in HIT testing. Finally, they will be able to apply diagnostic strategies to clinical scenarios.
